The Assessment/Intake is the central point of admission to all programs. Screening and an assessment are provided to all individuals seeking care for themselves and or family members. Clients are assessed and referred to appropriate services within the organizational system of care. Services are also provided on a walk-in basis. At intake, you will need to bring proof of income, a government issued identification, and a list of all current medications.
